Proposed therapy based on lung injury stage at the time of treatment
- No severe reduction of compliance
- Presence of oedema and protein in alveoli
- Improvement of gas exchange
- Prevents progressive lung disfunction
Notes:
Precocious treatment, particularly in the presence of still homogeneous lung pathology and with normal compliance, allows the use of reduced doses and prevents progressive deterioration of the lung
Late intervention and when compliance is reduced, are situations requiring higher doses to balance the inhibitory effect of the proteins present in the alveolar oedema. Repeated surfactant supplementation may be necessary to improve the lung pathology, often obtaining only improvement in gas exchange without curing the lung pathology completely.
