Conditions in which the lungs are completely different before and after re-constructive surgery. Diaphragmatic hernia may present either on the left or right, and involve the lung to differing degrees. If the presence of viscera is precocious in pleural cavity, the lung may be agenesic and future expansion therefore difficult. Neonatal ventilation at birth must be carried out with great care to avoid lung rupture (interstitial emphysema, pneumomediastinum and pneumothorax). Surgical repair generally is performed when the newborn is stabilized because in this condition the final results are more satisfactory.
